Physical Exam Form

*** A physical and dental exam is required to be completed by August 15, 2022.  If a student has had a dental or physical exam within one year of enrollment into Kindergarten, then BAEC will need a copy of the exams from the MD or dentist.  Copies can be faxed to 724.947.8143.  Thank you.
